Invisalign® Informed Consent and Agreement
Introduction
Your doctor has recommended Invisalign® for your orthodontic treatment. While this treatment can enhance your smile and oral health, it is important to understand its limitations and potential risks before beginning.
Device DescriptionInvisalign® aligners, developed by Align Technology, Inc., consist of a series of clear, removable plastic appliances designed to move teeth gradually. Your doctor’s prescription and advanced computer modeling are used to create a custom treatment plan, which determines the movement of your teeth throughout the process.
Procedure
Before treatment, you may undergo an orthodontic evaluation, including X-rays and photographs. Impressions of your teeth are taken and used to create a 3D ClinCheck® model of your proposed treatment. Once approved, a series of custom aligners will be manufactured and provided to you. Each aligner is numbered and must be worn for 20 to 22 hours per day, except when eating, drinking, brushing, or flossing. Typically, aligners are switched every 1-2 weeks under the guidance of your doctor. The total treatment time varies based on complexity. Follow-up visits will be scheduled approximately every 6-8 weeks. Some patients may require additional aligners, attachments, or elastics.
Benefits
1. Nearly invisible aligners for a discreet treatment experience.
2. No metal wires or brackets.
3. Removable aligners allow for easier oral hygiene.
4. Custom treatment planning via the ClinCheck® software.
5. Potential improvement in periodontal health.
Risks and Considerations
As with any orthodontic treatment, Invisalign® has certain risks:
1. Non-compliance – Failing to wear aligners as directed may extend treatment time and affect results.
2. Discomfort – Tooth soreness and minor irritation to the gums, cheeks, or lips may occur.
3. Relapse – Teeth may shift after treatment if retainers are not worn as instructed.
4. Oral Health Issues – Poor hygiene may lead to tooth decay, gum disease, or white spots on teeth.
5. Speech Impairment – A temporary lisp may occur but typically resolves within a few days.
6. Excess Saliva/Dry Mouth – Some patients may experience these side effects.
7. Attachments – Small tooth-colored attachments may be bonded to teeth for improved movement.
8. Bite Changes – Your bite may shift during treatment, potentially requiring adjustments afterward.
9. Additional Procedures – Some cases may require additional orthodontic treatments, such as elastics, interproximal reduction (IPR), or refinements.
10. Black Triangles – Patients with previously crowded teeth may develop small gaps near the gum line after alignment.
11. Pre-existing Conditions – Teeth with past trauma, large fillings, or dental implants may respond unpredictably to Invisalign® treatment.
12. Root Resorption – In rare cases, orthodontic movement may shorten tooth roots.
13. Retention – Wearing retainers after treatment is essential to maintaining your results.
